Before participation in any Capital Soccer program, participants and guardians recoginize that they understand and accept The Capital Soccer Incorporated waiver associated with the program.  This includes the participation, medical release, and COVID-19 waiver forms (all located on the Capital Soccer website).  Players are not permitted to participate in a program until Capital Soccer Inc is in receipt of this submitted form.
