Growing Hope Field Trip Request Form
Thank you for your interest in visiting the Growing Hope Center. Growing Hope's Field Trip Program aims to bring youth, from pre-k through high school, to the Growing Hope Center to experience an urban farm first-hand. Youth will have the opportunity to participate in preparing healthy foods, nutrition education, planting, harvesting, and eating local, seasonal produce. It is our goal that the field trip program is an educational and enjoyable experience for the participants.

Please answer the following questions in order to help us plan a field trip that suits your group.

The fee for field trips is $3 per youth participant. Funding is available for groups where at least 50% of participants are eligible for food assistance benefits, such as SNAP or free/reduced school lunch.

Timing of Field Trips
Growing Hope will host field trips lasting from 1-2 hours. Depending on your activity requests and your schedule, we will prepare a field trip that best fits your needs.

We desire at least two weeks advance notice from you to prepare your field trip.

Role of Visiting Organization
We request that a Growing Hope photo release form be distributed to participants’ parents/guardians and that upon arrival a list of students who have a signed photo release form is given to us along with the signed forms. Signing the photo release is completely optional and will not impact participation in the program.

We request that the visiting organization have a permission to participate form signed by parents/guardians.

During the field trip, chaperones should have access to emergency information for each participant including family contact information and relevant medical information.

The visiting organization is responsible for ensuring all participants have any medication that they may need. Growing Hope will not store any medications.

The Role of Chaperones
Chaperones are expected to assist Growing Hope Staff with supervising and managing the behavior of participants.
Appropriate Clothing
Remember that we will be learning and working outside. Please wear closed-toed shoes and dress appropriately for the weather.

Cancellation of Field Trip Program
If you need to cancel the program please give us at least two days in advance notice.

Growing Hope will cancel the program due to inclement weather at our discretion.

Facilities Information
The majority of our facilities are accessible for persons with mobility impairments. If accommodations are needed to make participation easier, please let us know prior to arrival.

Parking at the Growing Hope Center is limited, street parking is available across Michigan Ave on 1st Ave.

Transportation
Your organization is responsible for transporting the participants safely to and from the Growing Hope Center. It is recommended that your organization have an emergency transportation vehicle.
