2020 Indoor Worship Registration

Registration is required for indoor in-person worship services during the fall of 2020 at St. Andrew’s Episcopal Church, Glenwood, due to the COVID-19 pandemic. If you would like to invite a friend or neighbor to worship, please forward this form to them. If a decision is made to cancel in-person services, the 9 am online worship service will remain available on Facebook Live (www.facebook.com/standrewsglenwood/).

I will remain 6 feet away from people who are not in my immediate household at all times,

wear a face mask at all times,

practice good hygiene and use hand sanitizer when entering and leaving the church building,

stay home if I exhibit any COVID-19 symptoms (including a fever of 100.4 degrees or higher, cough, shortness of breath , chills, fatigue, headache, sore throat), or have been exposed to someone who has tested positive for COVID-19 within the past 14 days,

refrain from any physical contact with persons outside of my immediate household while at church.

I understand it is strongly encouraged that individuals consider not participating in in-person church programs at this time if immunocompromised.

I agree to requirements above and to abide by the Center for Disease Control’s (CDC) recommendations for the prevention of the spread of the 2019 Novel Coronavirus Disease (COVID-19) and other communicable diseases, and I attest to having read the Diocese of Maryland Covenant for Regathering at https://images.yourfaithstory.org/wp-content/uploads/sites/6/2020/05/20184353/Covenant-for-Regathering-5_20.pdf and the CDC’s guidance at: https://www.cdc.gov/coronavirus/2019-ncov/prepare/prevention.html. I assume all such risks being known, appreciated, and accepted by me. I agree to abide by any decision of church staff and volunteers regarding safety and protocol. I assume all risks associated with gathering in person, all such risks being known and appreciated by me. Having read this waiver and knowing the facts in consideration, I, for myself and anyone entitled to act on my behalf, waive and release St. Andrew’s Episcopal Church, Glenwood, church staff and volunteers, other participants, their representatives and successors from all claims or liabilities of any kind arising out of my participation in this activity even though that liability may arise out of the negligence or carelessness on the part of the persons named in this waiver. Further, I grant permission to all of the foregoing to use any photographs, motion pictures, or any other record of this activity for any legitimate church purpose.
