COVID-19 Proof of Vaccination Upload for Chaminade Students
This form is no longer accepting responses. To upload your COVID-19 vaccination card, please go to https://www.chaminadeuniversity.sharepoint.com (this is the former Chaminade Portal). You will need to login to SharePoint Online for the first time.
- Enter your Chaminade email address. The password will be your 7-digit CID account’s password.
- Upon initial login you will be asked to setup additional security information for multi-factor authentication to keep your account secure. Please follow the on-screen directions to complete then verify the security setup.
- Choose your authentication method - Cell Phone (recommended) or Land Line.
Accessing COVID-19 Forms:
- Go to https://www.chaminadeuniversity.sharepoint.com and login
- Click on the “Students” link
- Based on your needs select either the “COVID-19 Vaccination Card Upload” or the “COVID-19 Test Results”
- Click on the “New” button to begin your form upload.
