Facility Use Request Form
All requests must be submitted at least two weeks prior to the activity. Submission of your request does not guarantee approval. You will receive email notification of your approval or denial. Any questions regarding this form should be sent to Stacey Cory in the District Office - scory@oahornets.org or 585.948.5211x4291

NOTE: Community organizations and non-school groups must provide a valid certificate of insurance prior to facility use requests being approved.

The use of buildings excludes any audio visual or computer hardware/software unless training is received. Please send an email to tech.help@oahornets.org to request training.
