Student Device Request Form
This form is for reporting student devices (Chromebook or iPad) that are not functioning properly, as well as requesting a new device for home use. After completing the form please report to HS North on Tuesday, Thursday, or Friday from 11 a.m. to 2 p.m., this form must be completed prior to reporting to HS North. If you are reporting a damaged device, please bring the device with you to HS North.
