Our Prayer Chain ministers commit to praying for your requests shared, treasuring the sensitive and sometimes confidential nature of the requests, and respecting the vulnerability of the people in need.

Please provide updates by submitting a new form and indicating that it is an update for an existing request. If no update is received, a minister will follow up with you regarding your request in six months.

Requests are removed from the prayer list after six months. You are welcome to re-submit an additional request at that time.

If the parishioner prayer recipient would like a visit from a communion minister, parish life minister, or anointing of the sick, please contact the Parish Office at 502-245-9733 to make arrangements. (*Please note that completing this form will not automatically submit a request for visitation or anointing.)
