TWU COVID-19 Vaccine Self-Identification
This form allows members of the TWU community to identify themselves as meeting the Phase 1A, 1B, and 1C criteria under the state of Texas vaccination priorities. Student Health Services will use this information to contact individuals if and when additional doses have been allocated to TWU.

The phases are defined by the Texas Department of State Health Services as follows:

Phase 1A: Health care workers and support staff, EMS providers, pharmacy staff, public health and emergency response staff involved in COVID-19 testing or vaccination, and mortuary or death service providers.

Phase 1B: People 65 years of age and older, people 16 and older with a chronic medical condition that puts them at increased risk for severe illness from COVID-19, and teachers, school staff, and childcare workers.

Phase 1C: People 50 to 64 years of age.

Phase 2: All individuals 16+

Completing this form does not guarantee that you will be offered an appointment to be vaccinated. Because vaccine supply continues to be limited, decisions about which groups to prioritize within the DSHS defined phases will be based on the risk of direct exposure to COVID-19, risk of serious illness and death due to age and other factors, and the national priority to reopen schools.

As Student Health Services works to serve the needs of the TWU community, please do not call their office about appointments. You will receive an email from Student Health Services if and when an appointment is available.
