For volunteers age 17 or younger, the following affidavit is required to be read by the volunteer applicant and his/her legal guardian, and signed by the legal guardian.
This affidavit covers any Camp Hope-sponsored in-person events between January 1, 2026 through January 31, 2027, including but not limited to overnight summer camp located at Camp Red Leaf/Camp Henry Horner (owned/operated by Camp Nageela Midwest), and seasonal events.
Anchored in Hope, dba Camp Hope is a 501(c)(3) non-profit organization and is hereby referred to as Camp Hope.
