ONLINE | Registration Form

This form is for live online Certified Food Protection Manager (CFPM) training with Laura Finley.  

Submit one form per attendee. Managers registering multiple employees should submit a separate form for each attendee, then make one Stripe payment for the total number of registrations.

The training fee is $200 per attendee. The certification exam is purchased and scheduled separately. After payment, the registration contact (person filling out this form) will receive purchasing and exam scheduling instructions, and each attendee will receive class details and support throughout the certification process.
