Fine Arts Showcase Shout-Out Order Form
Your shout out will NOT be printed in the program without payment.
$10 each

Shout-outs must be paid by November 30.

Payment instructions:
1. Put payment into a sealed envelope, cash or check made to PBPA
2. Label the envelope with "Fine Arts Showcase Shoutout" and Purchaser's Name as it appears on this form.
3. Have your student drop it in the band room payment box; or drop it in the front office with attn: Josh Rudolph

If your envelope is not labeled properly, your shout-out may not appear in the program. No refunds.
