The Emilio Aguinaldo College (EAC) conducts a Graduate Tracer Survey (GTS) to assess EAC graduates’ employability and eventually, improve its program offerings.  This survey has adopted questions from the CHED Graduate Tracer Survey.

You have been chosen as a respondent of the EAC GTS. Ethical considerations regarding research on human subjects require that you sign this consent form.

Consent:

I understand that my participation as a respondent in this EAC GTS is not compulsory and that I can withdraw my consent at any time without penalty and have the results of my participation, to the extent that it can be identified as mine, removed from the records.

I agree to participate as a respondent in the EAC GTS. I also agree to participate in possible follow-up interviews that may be undertaken.

I understand that the results of my participation in this survey as well as in possible follow-up interviews will be held in the strictest confidentiality and will not be released to anyone other than the EAC research team in any individually identifiable form without my prior consent.
