Only students whose neighborhood school is on the state Open Enrollment list, and who have attended that school for at least one school year, are eligible to apply for enrollment using this form. To see if your neighborhood school is on this list, check:
Applications will be accepted until May 2, 2016 at 4:00 pm. If applications exceed available space for a school, a lottery will be conducted to determine which applications are accepted for that school.
If accepted into a school through Open Enrollment, it is the parent's responsibility to provide transportation to and from the school of choice.
Applicants must meet enrollment criteria as established in Board Policy 5118.
