Online & Digital Communication Consent Form - Ages 12 & Under
Dear Parents,

We are committed to conducting safe Kids and Youth Ministry during these unprecedented times where online contact has become one of the only forms of interaction. We’re aware that the online space has great advantages as well as some risks that we need to manage well.
 
Horizon Church has a Child Protection Policy and adheres to the Child Protection policy of Australian Christian Churches. This remains in place during this season of 'remote' ministry.

As we partner with you to care for and protect your child, our aim is to clearly communicate how we intend to use online/phone contact to interact with and support your child.
 
We’ve started to use online/non face-to-face tools to stay better connected with our Children. These include (but are not limited to):

    -  Zoom groups
    -  Phone calls (via parent's contact)
    -  Instagram Live
    -  YouTube and Facebook Live (including live comment interaction - Mainly Horizon Youth)

In addition to our Child Protection Policy and the requirement that every Staff member, Youth and Kids leader hold a current Working With Children Check, our Staff, Youth and Kids team have read and agreed to our “Safe Ministry to Children and Young People: Online & Digital Communication Mediums” conditions. We have provided a copy of this with this consent form.

To summarise the main points:

    -  Our leaders will always contact you about any online activities. Please do your best to acknowledge their communication as it will help them know you have received the information and are supporting the Kids and Youth Ministries.
    -  Our leaders will maintain a safe environment to the best of their ability.
    -  Our leaders are trained to inform us of any concerns that arise during online activity 
    -  Our leaders want to help your child grow in their walk with God and support them as they live their  everyday life, safely using online and digital platforms.

I being the parent/guardian of the said child/ren hereby give my consent that my child/ren may participate in Horizon Church’s Online Program.

I am aware that our Kids and Youth Ministry leaders will inform me about Online Program times and will moderate the interactions.  Some online programs with children will be recorded for accountability and transparency for child protection reasons. These will be securely stored at our church for 45 years.

Bullying, harassment, discrimination, and inappropriate language and conversation or visuals will not be tolerated at any time and the failure by my child to follow these standards may result in their exclusion from future activities. I agree to discuss online safety with my child, including protecting their personal contact details and location from online forums.

I understand that I may be held responsible for any comments made by my child if I allow the use of my own personal social media or Zoom account for my child to participate. I will disallow my child to comment or interact in adult forums (eg. Sunday Church Services online)

I agree to supervise my child at all times and take full parental responsibility for their participation.
