Registration Form
STEP 1:  Register 
  1. Visit our website to view the dates of the prep sessions:  www.beechwoodactprep.com
  2. Complete this registration form (below) to let us know which class you would like to attend and to provide us with important information about you.
STEP 2:  Check your email
  1. AFTER you complete this form, you will receive a registration confirmation email from "Google Forms" as the sender and with "Registration Form - Beechwood ACT Prep, Inc" in the subject line.
  2. Choose one of the payment options from the completed registration form.
  3. If you don't see an email from Google Forms or an email from info@beechwoodactprep.com, check your "Junk" or "Clutter" or "Updates" or "Social" or "Primary" folders, especially if there are restrictions on your email server.
STEP 3:  Make Payment 
  1. Submit an electronic payment via Venmo (@Julie-Rash-1).
  2. Mail a check (Beechwood ACT Prep, INC 6254 Streamside Drive, Independence, KY 41051). 
  3. Mail a money order (Beechwood ACT Prep, INC 6254 Streamside Drive, Independence, KY 41051).
  4. Email us if you plan to bring cash to class (info@beechwoodactprep.com).
  5. Email us to arrange a payment plan (info@beechwoodactprep.com).
STEP 4:  Check your email
  1. Please note that your payment secures your spot.
  2. AFTER your payment is received, you will receive a payment confirmation email from info@beechwoodactprep.com, confirming your spot in the class, along with location, materials, class procedures, and important deadlines.
  3. If you do not receive a payment confirmation email within 48 hours of making your Venmo payment or within a reasonable time for postal service, then please contact us via phone call or text:  859-462-3414 or 513-646-3659. 
Prices include test prep for the following subjects and all materials (including book):
  • $535 - Prep for the Standard ACT (English, Math, and Reading)
  • $585 - Prep for the ACT with Science Add-On (English, Math, Reading, and Science)
