The safety of our students and employees is our priority. As the coronavirus (COVID-19) pandemic continues, we are monitoring the situation closely and following guidance from the Centers for Disease Control and Prevention and local health authorities.  In order to prevent the spread of the COVID-19 and reduce the potential risk of exposure to our students and workforce, we are asking everyone that's feeling ill to complete and submit this questionnaire to report employee's COVID-19 illness or COVID-19 in the home. *Please follow the guidance given at the end of the questionnaire.* Please respond to each of the following questions truthfully and to the best of your ability.  Your participation is important to help us take precautionary measures to protect you, our staff, and our other students.
