LEAF Evaluation Application Form
LEAF (Leaders in Environmentally Accountable Foodservice) aims to become the national leader in sustainable foodservice standards, and to provide establishments of all sizes with the knowledge, tools and confidence to move forward in their environmental efforts, while supporting successful business practices. Being LEAF certified demonstrates that a foodservice establishment has met rigorous environmental criteria.

The LEAF evaluation and certification program was developed as performance standards for the operations and practices of foodservice establishments, including but not limited to: independent restaurants, chains, and institutional foodservice operations, public and private. It has grown to include cafes and food trucks.

Where possible, LEAF has strived to utilize complementary third-party verification and labeling systems that implement rigorous data collection methods, and published and peer-reviewed reporting.

Certification Levels (valid for 3 years each)

Level 1 Certification:
The establishment meets the minimum requirements for certification with LEAF, and has made a positive attempt to become an environmentally-sensitive foodservice establishment.

Level 2 Certification:
The establishment meets above the minimum criteria required for certification with LEAF, and has proven to be committed to reducing the impact of the establishment on the planet. This establishment is well on its way to reaching top level status for sustainable foodservice.

Level 3 Certification:
The establishment has reached the top level of environmental sustainability. The establishment makes a continuous effort to reduce environmental impact, and contributes above and beyond that of other establishments to provide the most sustainable foodservice experience to its patrons.
