Please join us for the Oak Park High School Girls' Basketball 2-Day Clinic on Saturday, June 13 and Sunday, June 14, 2026, 9:00 am to 12:00 pm. 

To register, please fill out the information below for your player   

Cost for the clinic is $250 and focuses on building confidence, improving fundamental skills, and fostering a love for the game from the Back-2-Back-2-Back-2 Back Champions, the Oak Park Lady Eagles.

 Participant Release Waiver of Liability
This Release and Waiver of Liability (“Release”) is given by the undersigned clinic participant (or the parent/legal guardian on behalf of such participant) on behalf of his/her heirs, executors and representatives (the “Participant”) in favor of Oak Park High School Girls' Basketball Fundraiser Program and each of its sponsors, partners, affiliates, employees, team members, volunteers and recipients of services (collectively, “Oak Park High School Girls' Basketball Fundraiser Program”).

WAIVER, RELEASE AND INDEMNITY. In consideration of being permitted to participate in an event/camp/clinic/performance affiliated with, sponsored or hosted by  Oak Park High School Girls' Basketball Fundraiser Program (the “Basketball Clinic”), Participant hereby agrees to forever release, waive, discharge, and hold harmless all  Oak Park High School Girls' Basketball Fundraiser Program from any and all damages, claims, actions, liability, demands and costs, including attorney fees, resulting from Participant’s participation in the Basketball Clinic, including without limitation for or from any illness, accidents, injury or death to Participant or any other person, or injury to any property.

ASSUMPTION OF RISK. Participant acknowledges and agrees that (1) the  Oak Park High School Girls' Basketball Fundraiser Program may include activities that may be hazardous to the Participant, (2) Participant’s participation is voluntary, and (3) Participant hereby expressly and specifically assumes the risk of injury or harm, whether caused by any act or omission, including negligence, of  Oak Park High School Girls' Basketball Fundraiser Program or otherwise.

MEDICAL TREATMENT AND NO INSURANCE. Participant understands that Oak Park High School Girls' Basketball Fundraiser Program does not carry or maintain health, medical, or disability insurance coverage for any Participant. Participant does hereby release and forever discharge the  Oak Park High School Girls' Basketball Fundraiser Program from any claim whatsoever which arises on account of any first aid, treatment or emergency service rendered in connection with Participant’s activities in the Basketball Clinic.

Participant (or his/her parent or guardian, if under age 18) has read this Release, fully understands the terms of this Release, and understands that Participant is giving up substantial rights, including the right to sue. Participant is signing this Release freely and voluntarily, and intend by such signature to be a complete and unconditional release of all liability as provided hereunder to the greatest extent allowed by applicable law. Participant agrees that this Release is intended to be as broad and inclusive as is permitted by the law, and that if any portion of this Release is held invalid or unenforceable, all other terms or portions of this Release continue in full legal force and effect to the fullest extent permitted.
