PAYMENT TERMS & CANCELLATION / REFUND POLICY
Once registered, Erie Premier Sports will send the registered main contact an online payment link, to pay using a credit or debit card. If a payment link should be sent to another contact please email Russell Segesdy directly at russell@eriepremiersports.com  A $1000 deposit is due upon registration for U13-U19 teams.  Balance is due by November 1, 2024.  All U9-U12 teams must pay in full upon registration.

I understand and accept that all teams must complete an Erie Premier Sports Roster/Waiver Liability Form and Cool Springs Roster/Waiver Form.  We do not need the form every time, it just needs completed once.  Completed forms can be send directly to Russell Segesdy at russell@eriepremiersports.com or handed to the manger on duty.

I agree and understand that once a team is accepted there are no refunds.
