I, the parent/legal guardian of the above named youth, give approval for participation in The First Tee sponsored activities. I assume all risks of injury and agree to hold harmless The First Tee of the Upstate., The Golf Club at Botany Woods, Shanks Driving Range, Greer Golf , 3s Greenville -Cross Winds Golf Course, Greenville Junior Golf Land, Furman Golf Club, The Carolina Country Club, Carolina Springs Golf Club, Woodfin Ridge, Cherokee National, Willow Creek, Thornblade CC, Shanks Driving Range, Haas Family Golf, The Preserve at Verdae, The Club at Brookstone, Cherokee Valley Golf Club, or Headquarters Office from claim(s) of any nature arising from any activity, including transportation, connected with The First Tee facility or program. This hold harmless agreement includes, but is not limited to, any claim due to injury resulting from negligence of The First Tee of the Upstate, The Golf Club at Botany Woods, 3's Greenville Golf Course, Greenville Junior Golf Land, Furman Golf Club, The Carolina Country Club, Carolina Springs Golf Club, Woodfin Ridge, Cherokee National, Willow Creek, Thornblade Club, Shanks Driving Range, Haas Family Golf, The Preserve at Verdae, The Club at Brookstone, Cherokee Valley Golf Club, TopGolf, any off site First Tee related event, or Headquarters Office, its employees, agents, participating agencies, and volunteers. I consent to The First Tee of the Upstate and Headquarters Office communicating information regarding my child’s participation via the Internet and give permission to use photos and or video images of the above mentioned minor for lawful promotional purposes.      

I also understand that First Tee Upstate SC events, classes, or camps may be cancelled due to weather, we will try to reschedule but in most cases due to facilities schedule this may not be possible.

Symptoms of COVID-19 include: • Fever • Fatigue • Dry Cough • Difficulty Breathing I agree to the following: • I understand the above symptoms and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above within the last 14 days. • I affirm that I, as well as all household members, have not been diagnosed with COVID-19 within the past 30 days. • I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days. • I affirm that I, as well as all household members, have not traveled outside of the country or to any city considered to be a “hot spot” for COVID-19 infections within the past 30-days. • I understand that the First Tee Upstate South Carolina cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.
