Thank you for your interest in signing up to a NEL Music Centre group! This consent form allows you to attend any of the groups we run here at NEL Music Centre. You can sign up to as many groups as you like,  a consent form is required to be completed each academic year in order to maintain your membership. We need some information about each child / young person that would like to join our groups so please complete one consent form per child. There are 3 short sections to complete.
