(1) Minimum of 6 students for each session. Students must be at least 8 years old.

Summer Mao is a USA Badminton certified coach specialized in youth training. She was a professional player in Shanghai Team in the 1980-1990s. She still plays competitive badminton regularly and competes in tournaments nationwide. Her coaching experience includes serving as skill coach and instructor for the Burnsville High School and Edina High School varsity badminton program, Yinghua Academy, MN Huaxia Chinese School, and Minnesota International Chinese School. She also offers regular and summer group training sessions and private lessons to youths ages 8-18. Her coaching programs focus on beginners and intermediate players. Some young players she trained competed in and won some regional and national tournaments. She has completed Minnesota State High School League's Coach CER chapters 1-4 and received the certificate. She also completed the Sport Performance and Safety Training offered by USA Badminton and became a certified coach.

In consideration for myself and/or my child being allowed to participate in the subject activity, for myself, my heirs and personal representatives, to the extent allowed by law, I hereby waive and release all claims for damages I or my child now or may hereafter have against BloomingBirds Badminton, Burnsville High School, Bloomington Public Schools and their affiliated facilities, and their agents and for any injuries and damages suffered in connection with my or my child's participation. I further agree to defend, indemnify and hold harmless BloomingBirds Badminton, Burnsville High School, Bloomington Public Schools and their affiliated facilities, and their agents from all claims for injury or death, or for loss or damage to property, filed by anyone against BloomingBirds Badminton, Burnsville High School, Bloomington Public Schools and their affiliated facilities, and their agents which arises out of my or my child's participation, except for injury or damage caused by the sole negligence of BloomingBirds Badminton, Burnsville High School, Bloomington Public Schools and their affiliated facilities, and their agents. I, the undersigned participant and/or parent or guardian of the minor participant, give my permission to have photos/video tapes taken, without recompense, during activities and used for publicity purposes.

By typing my name  in the box below, I am effectively providing my signature, indicating that I agree to the "Hold Harmless Agreement" and understand that all the information I entered on this form is true and accurate and that my registration is considered complete only when my payment is received.
