Service name(s) *
The common name of the affected service or group of services. Even if all services are affected, please specify the individual relevant public facing services.
Activity/issue *
A single sentence describing the work that will be carried out (and why) or other cause of the downtime, e.g. "Migration from server X to server Y" or "Upgrading application Z to version 1.2.3".
Impact *
Brief description who/what is expected to be impacted by the downtime and how. Sometimes it may also be valuable to explicitly specify who/what will not be impacted (e.g. end-user can still access the service but cannot login and no changes to the data can be made).
