PARENT AGREEMENT:

(Please type students first and last name below) has my permission to attend the Middle School Dance on Friday, November 18th, 2022. My child will abide by all of the school rules as outlined in the Student Handbook.

I understand that my child must remain in the school building for the entire duration of the dance, 7:30 – 9:30 PM.

I accept responsibility for retrieving my child from the dance in a timely fashion at the close of the dance, 9:30 PM. I will wait for my child in the foyer or outside the building until students are dismissed.

I understand that my child is subject to the consequences for misconduct at the Middle School Dance. If my child is removed from the dance, I agree to come and pick him/her up and I can be reached between 7:30-9:30 pm at the following phone number: ___________________(please type phone number below)

I understand that misconduct may result in my son/daughter not being able to participate in future dances this academic year.
