This form is to be completed by the nominating athletic trainer, who shall be responsible for the completion of all sections of the application by February 5, 2025.

CRITERIA FOR APPLICATION
The Washington State Awards Committee and Board of Directors seek to recognize a physician and his/her organization that support the athletic training profession and athletic trainers.

This award will be presented at the WSATA State Meeting at the District 10 Annual Clinical Symposium in March.
