Each RUSD parent/guardian has the right to determine if a library resource or collection is right for their child and should accord the same right to other families.

By completing this form, a parent/guardian is requesting that their child not be able to check out a specific title or library resource (see Section 2).

You may also use this form to restrict your child's access to their classroom library (see Section 3).
