Form Instructions
This form must be completed by the owner/guardian of the pet(s). This form has three sections: Owner/Guardian Information, Pet Information, and Important Blue Sparrow Information and Policies. This form is designed to accept up to six pets. If you have more than one pet to enter, please enter one pet's information at a time. After submitting information for one pet, you'll be prompted to add additional pets if needed. Please be as thorough and complete as possible. If you submit this form after-hours, over a weekend, holiday closure or day we are not available, please allow 3 business days for a response upon our reopening. Disclaimer: Practice Limitations & Scheduling Policies
As a small, San Francisco-based, housecall-only practice, we are unable to offer on-call, same-day or emergency services. All appointments require a minimum of one business day's advance notice and are dependent on appointment availability. If your pet is experiencing a medical emergency, please contact your nearest veterinary emergency hospital immediately.

As our practice is based in San Francisco, appointments in our service areas outside of the city require a significant scheduling lead time, typically several days to several weeks in advance. Due to this constraint, we are unable to offer adequate palliative and hospice care for patients residing outside of San Francisco. Palliative and hospice patients often require care within 1-2 days, which our scheduling model for areas outside of San Francisco cannot accommodate. Furthermore, we understand the difficulty of scheduling end-of-life care far in advance. Our required lead time for humane euthanasia appointments outside San Francisco may not be suitable for the often urgent nature of this need.

At this time, we are unable to see fractious, aggressive, or defensive pets, as well as pets that are extremely fearful or difficult to handle.

San Francisco, Sausalito, Berkeley, Emeryville, Oakland, Piedmont, Alameda, South San Francisco, Brisbane, San Bruno, Broadmoor, Daly City, and Colma. We have suspended service for Pacifica for the foreseeable future.

We are no longer routinely practicing COVID protocols during appointments, but will gladly accommodate individual requests to practice COVID safety protocols during appointments. If you would like our staff to practice any COVID safety protocols (masking, socials distancing, abstaining for touching doorknobs and surfaces, etc), please include those specific instructions below. (Our staff may temporarily practice COVID certain safety protocols during periods when there are a high number of COVID cases. So you may sometimes see our staff wearing masks during visits, using hand sanitizer, or requesting to wash their hands upon arrival.)
