Request for Transportation to Non-Public School Application - Monroe Local Schools
If you are a Monroe Local School District Resident and have a student(s) who attends a nonpublic or community school please complete this information to request transportation (to a non-public school that is within the 30 minute radius of our district school of attendance by bus at the time of day when the trip would occur) below so that a determination of eligibility and practicality may be made based on the standards in our policy and ORC 3327.

Please complete a copy of this form for each location / school if you have multiple.

Additionally, you will need to verify your residency on Final Forms by uploading a current utility bill and a proof or residency (Lease, deed, mortgage statement, etc).  Log on to the Final Forms system from our district website at https://www.monroelocalschools.com/departments/enrollment/enrollment-and-annual-forms/ create a Final Forms account and indicated your student is an 'External Candidate'.
