Aggressive Behavior Incident Report Form
FCHS is committed to providing our students with a safe school environment. FCHS policy states that, “Aggressive behavior of students of the District shall not be permitted.” Policy defines aggressive behavior as: “any behavior that may cause physical or psychological harm to someone else and/or urging other students to engage in such conduct.” (Student Handbook, p. 27)

FCHS recognizes that coming forward with information about bullying or aggressive behaviors is difficult, but we strongly encourage those who are victims, witnesses, or otherwise aware of bullying to notify school personnel. If you prefer, we have provided an “incident report form” that can be submitted online. This report form will be directly submitted to Mrs. Van Gundy in the guidance office and will be addressed within 24 hours or the next business day. YOU MAY REMAIN ANONYMOUS, but it is much more helpful to the school if we can contact you for additional information and clarification.
