If your child(ren) have been previously enrolled in the LINK Program (launched Sept. 2019), please use this form to update their information.

After completing this form you do NOT need to re-complete the full enrollment packet that you completed when you first enrolled your child(ren) in the LINK program; however, we need to have updated physical and immunization records on file for each child in the program.
