Please fill out this survey once for each of your children who will be riding a Whitehall school bus during the school year. (even if it's just "from Lighthouse to Shoreline in the morning," etc.)

In order to reserve a seat on a bus for your child, you must have this form submitted at least 5 school days prior to his/her expected first date of riding the bus.

After receiving your survey response, we will attempt to assign your child(ren) to the nearest bus stop to your home/daycare. If the current closest bus stop is on a bus that is full, we will assign the next closest bus stop on another route. You will be able to see your child's assigned route & bus stop location/times by looking on PowerSchool under the Student Profile beginning August 12th.

THIS form is for all NON-KINDERGARTEN/ECSE/YOUNG FIVES students.
