Please note that this form is not an official form for the U4U program but a pre-screening tool that our volunteer legal & logistical teams at the Consulate and the Law Firm can use to facilitate beneficiary-sponsor matches as a public service to US sponsors and Ukrainians who reached out to us requesting such matches.

EXPLAINING THE U4U PROGRAM:

They are instead granted a humanitarian parole BASED SOLELY ON SPONSOR'S INDIVIDUAL SPONSORSHIP, allowing them to live - and upon applying for work permit, to work - in the U.S. for up to 2 years. The individual who is paroled into the US is known as a ‘‘parolee’’. This parole does NOT make one automatically eligible for refugee/ asylee or permanent U.S. status and parolee would have to consult with a licensed immigration attorney to determine how to secure such permanent legal status if desired.
Both sponsor and each beneficiary fill out our pre-screening form. We will try our best to match according to your collective requirements, taking into consideration number of people who could be hosted, genders, ages, schooling options for children, etc. We then notify the sponsor about the match and offer them the opportunity to speak directly to the beneficiaries in order for them to navigate personality fit directly and address any common questions and concerns.
Once sponsor and beneficiary confirm they agree to go forward with the match, sponsor fills out form I-134 FOR EACH INDIVIDUAL BENEFICIARY, EVEN IF FROM THE SAME FAMILY, with USCIS online via a MyUSCIS account (preferred to the paper filing because it is faster):

Form I-134, Declaration of Financial Support, requires information from both the sponsor and the beneficiary and we will be having such information available to the sponsor via beneficiary responses to our pre-screening forms.
There is no application fee for individuals applying for this program.
BENEFICIARIES of the Uniting for Ukraine program will need to:

Have resided in Ukraine as of February 11, 2022 and be displaced as a result of the war
•
Have a valid Ukrainian passport, or be included on a parent's passport
•
Complete vaccinations and other public health requirements
•
Submit to biometrics collection, biographic screening, and security vetting
•
Have a sponsor in the U.S. whose Form I-134 Affidavit of Support has been approved by USCIS.
Children under 18:

must be traveling with a parent/ legal guardian) who resided in Ukraine prior to Russian invasion (through February 11, 2022) and were displaced as a result of the invasion

who cannot travel with a parent to the US for this program but are coming to the US to meet a parent/ legal guardian may instead seek parole through a separate I-131 parole process
The I-131 parole process MAY NOT BE USED by prospective adoptive parents or to circumvent normal adoption processes.
