Thank you for partnering with Shady Strong to receive essential communication and mobility devices. Our mission is to help improve the quality of life for individuals and families impacted by ALS by providing the tools they need.

1. No Time Limit on Device Use:

Shady Strong loans out communication and mobility devices with no time limit. You and your loved one can use the device for as long as it is needed.

2. Loaned Equipment:

- I understand and agree that Shady Strong and its affiliates, sponsors, and vendors may, at their discretion, provide loaned equipment to me.

- I recognize that it is my (or my legal guarantor's) responsibility to ensure that the equipment is properly maintained and to seek repairs when necessary.

- Companies that manufacture assistive technology and equipment provide warranties specific to the product, and if the company requires registration of the equipment to activate the warranty, it is my (or my legal guarantor's) responsibility to complete that registration.

- I understand and agree to the policies concerning any warranty provided by the manufacturer as it relates to the specific equipment provided.

- I understand that the equipment remains the sole property of Shady Strong and must be returned when no longer being used by the person with ALS, so that it may be distributed to another individual in need. I also understand that if the equipment malfunctions and causes injury, Shady Strong cannot be held liable. Since this equipment is loaned at no cost to me, Shady Strong will be held harmless from any liability, misuse, or malfunctions of the equipment.

3. Care of the Equipment:

While the equipment is in your possession, we ask that you take good care of it to ensure it remains in optimal working condition for as long as possible. If you experience any issues with the device or require support, please reach out to us immediately.

4. Return Policy: In the event the person with ALS is no longer able to use the device, or if it is no longer needed, the device must be returned to Shady Strong.

5. Arranging Returns:

When it is time to return a device, please contact Shady Strong at (607) 725-4739 or via email at Kristin@shadystrong.com to arrange for the return of the equipment. We will work with you to make the process as smooth and convenient as possible.

6. Shady Strong’s Commitment:

All devices loaned through Shady Strong are provided at no cost to the person with ALS or their family. We believe in making these critical tools available to those who need them, and we rely on our generous donors and supporters to make this possible.

7. Monetary Support Available for ALS-related Expenses:

In addition to our equipment loan program, Shady Strong also offers monetary support to assist with a variety of ALS-related expenses.

This financial assistance can be used to help cover:

▪ Medical expenses, out-of-pocket medications or treatments: Helping cover co-pays or out-of pocket costs for ALS medications.

▪ Travel costs for medical appointments

▪ Co-pays for treatment and therapies

▪ Home improvements or modifications needed to accommodate ALS care, such as ramps, lifts, and accessible bathrooms. Vehicle modifications, such as wheelchair lifts and accessible vehicles.

▪ Caregiver support: Covering the costs of in-home caregivers or respite care.

▪ Assistive devices and home automation: Supporting the purchase of advanced assistive technology or smart home automation.

▪ Mental health services: Covering counseling and therapy for emotional support for the person with ALS and their family.

▪ Alternative therapies: Supporting complementary treatments such as acupuncture, massage, or speech therapy.

▪ Accessible travel for leisure or family visits: Supporting personal travel for visits or fulfilling lifelong wishes.
