Consent for Additional Dental Services
Once your pet is under anesthesia, your doctor will perform a complete oral exam and carefully check teeth and gums. During this comprehensive oral exam, your doctor may find one or more teeth that need to be extracted for health reasons. Dental x-ray(s) may be required to get an accurate diagnosis of the condition of your pet’s teeth and jawbone.
Your doctor or a veterinary technician will call you with an updated estimate if your pet needs any dental x-rays, extraction(s) or additional services. Based on the doctor’s recommendations, the following services may be added to your invoice before you pick up your pet:

Incisors each $10.00
Canine teeth (fangs) (each) $200.00
Premolar 1 (each) $40.00
Premolars 2 and 3 (each) $65.00
Carnasal (P4) (each) $140.00
Molars (each) $66.00
Deciduous teeth (each) $14.00
Dental roots (each) $32.00
Nerve block (each) $16.50
Root planing (each) $10.00
Doxirobe Application $68.00
Remove Hyperplastic gum mass (each) $16.50
Dental Radiographs first $30.00
Additional Dental Radiographs (each) $25.00
Should any unforeseen dental procedures be necessary and desirable in the veterinarian's professional judgement.

I prefer to be called before any additional procedures, other than emergencies. If I cannot be reached, I authorize you to proceed with all necessary dental procedures.
If I cannot be reached by phone, I do not authorize any unforeseen dental procedures.
