1. The course selection form is used to collect data on classes that students prefer. This information is then
 used to build our master schedule.

2. The information provided by students will also be used to create student schedules. We do our best to give students their top choices. However, we cannot guarantee that, due to teacher/course availability, scheduling conflicts, and class size.

3. Please complete all of the required information. Only submit ONE form. Any questions and/or changes to the form should be directed to your child's counselor, Mrs. Barnhart (A-K) or Mrs. Hill (L-Z).

4. Please complete this form, even if you are not planning to come to Evergreen next year. Until we know for sure that you aren't coming, we need to have your course selection form on file. Also, if your plans change over the summer, we will already have a schedule for you and you will be more likely to get your top choices.
