SFSC Facilitator Training Application Form 2017
The form has four sections. The first section asks for your contact details. The second section asks about how you are paying for the course. The third asks for information about your background and will be used to assess whether our services reach all the community and the fourth asks about future communications.

Your contact and payment details will be held on computer and will be used to process your application. The information about your background will be anonymised before being used for monitoring. All information will be held securely and will be confidential to the Race Equality Foundation.
