I would like to offer my services as a chaperone for Cedar Point. (Chaperones still must purchase a ticket for entry into the park for $40, or you can use a season pass).

We encourage you to use the EZ pay system! If you have a season pass, we will adjust your amount due once we receive this form. If you have requested extra tickets, we will adjust your amount due accordingly.
