Plum Baseball & Softball Association is committed to providing a safe, respectful, and positive environment for all players, parents, coaches, volunteers, and families.

This form is intended to provide a formal way to report concerns or incidents that may require review by PBSA leadership. Before submitting this form, we ask that you first attempt to address the issue directly with the team’s coach. If the issue cannot be resolved at that level, it should then be brought to the appropriate division representative or board member.

If the concern remains unresolved, you may submit this form to initiate a formal review.

Please understand that while PBSA will make every effort to handle concerns with discretion and respect, complete confidentiality may not always be possible if a full review is required. Providing clear details will help us properly review and address the situation.
