Spay/Neuter Voucher Application                        Limit 3 vouchers per year, per household.          Open from the 1st through the 15th of each month.
The Humane Society of Buffalo Trace provides vouchers to help Mason County, Kentucky residents cover the cost of getting their pets spayed or neutered. The vouchers are NON TRANSFERRABLE.                                    You must be a resident of Mason County Kentucky to receive a voucher!

To be considered for this program, please fill out the following form BEFORE your pet's surgery. (We cannot provide reimbursement for spays or neuters that have already taken place.) If selected, you will receive the voucher within one month of submitting your application. You will need to present the voucher to the veterinarian at the time of the surgery to receive the discount on your bill.

If you are requesting a voucher for more than one pet, please fill out a separate form for each individual animal.
The maximum is 3 vouchers per year, per household.
