Change of Transportation Form
Use this form to request a change to your student’s regular transportation. Please note that this form is for ongoing transportation needs—not one-time or temporary requests.

All transportation requests must:
  • Fall within the attendance boundary of your student’s school
  • Meet Board of Education criteria, as well as state statutes and regulations

Requests are handled in the order they are submitted and may take up to 5 days to process. You can track changes in the Parent Portal.
