Music Tour Chaperone Application

West High Music Tour Chaperones will be selected based on positive experience with Youth (including helping and with our own Music Students). Chaperones will partake in the activities and sight-seeing of the Music Tour, but will also have supervision duties, organizational duties, and could possible care for sick students, etc. Chaperones should have a good rapport with students, but also know that there is a time to be firm with upholding expectations. Chaperones should also be good role models of maturity and behavior.

Please adhere to returning this form by Sept. 13, so we know who is interested. We don’t know if you are interested if you don’t turn this form in! We are not exactly certain how many chaperones we will need at this point. If more than one person in a family wants to chaperone, please fill out separate applications.
