2023-2024 Florence Sawyer School Extended Learning Change Of Schedule Form and "Add A Day/Morning" Form
Welcome to the Extended Learning schedule change form! You may use this form to make a change to your overall schedule, or to all a single morning or afternoon.
Additions of AM care will be $8.00 and require a 48 hour notice and an email to the SITE COORDINATOR.
Additional days will be $39.75 per day and require a 48 hour notice. You will receive a confirmation email when your request is approved.
 
Please note there will be no 'switching days'. Any extra days needed will need to be added at $39.75.

All continuous schedule changes will result in a $15 change of schedule fee.
All schedule changes must be in effect for at least a month, or days will be added on an individual basis.

PLEASE NOTE ALL SCHEDULE CHANGES REQUIRE 48 HOUR ADVANCE NOTICE.
