Davenport faculty and staff members wishing to utilize Volunteer Time Off should submit this online request form at least 2 weeks prior to your requested time off. Supervisors (or a designee) will be responsible for approving all requests. Requests may be denied due to business needs, performance, etc. VTO or PTO may not be used if an employee has given notice to resign from the University.
