Family & Emergency Contact Information - Burley School
Burley School is required to have each family's contact information on file. This is essential for student safety and home-school communication.

ALL INFORMATION IS REQUIRED. This form must be completed by all families by the first day of school, Tuesday, September 5. After school begins, you will receive a printed confirmation to check, sign, and return.

This information will be kept confidential. At the end of this online form, you will have the option to give consent for your contact information to be included in a Burley PTA Student Directory.

Before you begin, be sure you have 1) two emergency contacts who live nearby and are NOT parents (friends, neighbors, etc.) and their names, addresses, and phone numbers, 2) your child's doctor's name, address, and phone number, 3) the room number or teacher's name for each child (on last year's 4th quarter report card), and 4) cell, home, and work phone numbers for both parents/guardians.
