Stevens Community Humane Society Volunteer Application
This application will serve as an official document to apply to be a volunteer with Stevens Community Humane Society.

Volunteer Requirements:

1. Be at least 18 years of age to work directly with the animals at the building and foster animals in own home. Ages 12-17 can participate in special events (parent or guardian maybe required to attend).

2. Participate in an hour long training session and attend as many monthly volunteer meetings as possible (scheduled by the Volunteer Coordinator).

3. Commit to scheduled volunteer shifts. Volunteers are asked to commit to at least 4 chore shifts per month for a minimum of 3 months. If conflicts arise, contact the Volunteer Coordinator.

Please note that some volunteer duties may require standing for long periods of time or lifting between 5 - 50 lbs.

Below is the Stevens Community Humane Society Waiver of Liability and Agreement to Indemnify (You will be asked to sign this in person before beginning an volunteer shift)

I, _________ understand that there may be risks involved with being a volunteer with the Stevens Community Humane Society; and hereby assume said risks and I hereby waive any and all claims which I may otherwise have against the Stevens Community Humane Society, because of injury to person or damage to my property resulting from any activity in which I may engage as a volunteer for said Society. I also assume full responsibility for my actions in service of Stevens Community Humane Society, and vow to respect the building and property.

To the best of my knowledge, I hereby affirm the below information given is true and give the Stevens Community Humane Society permission to use the information to obtain data regarding my background.
