I, Renee Miller, Math Specialist and Founder of Full Circle Math, agree to provide specialized academic instruction and academic coaching, for the student named below. I will collect data through assessment and student work, and develop a plan individualized to student and family needs. The Parent/Guardian named below agrees to the following terms:
Sessions will be 55 minutes in duration, unless otherwise agreed upon. Sessions will begin promptly at the agreed upon time. If the student is not present at the scheduled time, Renee will wait 15 minutes for them to arrive. After this waiting period, the session will be forfeited.
As other students may be scheduled immediately after your child’s session, if a brief parent consultation is desired, it must take place during the session period or be scheduled for another time.
During the Session: Parents will provide necessary space, tools, and technology needed for student to complete the session independently. The entire session may not be non-stop “work.” It is important for me to adjust to the student’s level of mental energy, fatigue and interest. Thus, parents will occasionally hear about breaks, walks, stretching, jokes, unrelated conversations, etc. These are a planned and expected part of educational coaching and specialized instruction.
Ongoing collaboration and communication between families and teachers is one of the key elements that supports student growth. I see us as partners and I aim to share what I learn about your child with you. To that end, I agree to provide either (a) one quarterly written progress report OR (b) one scheduled quarterly progress meeting by phone or in person. Each progress report/meeting explores the student's progress toward target goals and outlines their overall response to intervention.
Consultations with other professionals (by phone or in person) are charged at $120/hour, plus travel time (prorated to the nearest half hour). This includes meetings with a student’s teacher, school learning specialist, doctor, etc.

Rates: $120 per session. Reduced fee services are available on a limited basis and must be agreed upon in advance. Please let me know if this is outside of your family's financial means.
Payment: Cash, Check, or Venmo are accepted forms of payment. All payments must be made in advance of services rendered. Payment for your child’s scheduled sessions are expected at the beginning of each month. If you are unable to pay for all of your sessions at the beginning of the month, once agreed upon, you may pay on a weekly basis, prior to the start of each session. Cancellations: You understand that since the session time(s) have been reserved for you, you are responsible for the fee whether or not you receive services. However, I do realize that illnesses and emergencies do happen. Please be in communication as soon as possible for these unexpected cancellations.
For other cancellations, please A maximum of one session per month, with at least 48 hours cancellation notice, may be rescheduled without penalty. Fees will be forfeited for all other cancellations or no-show sessions.
Hold Fees: If your family needs to take a short break from their sessions for travel or other commitments, and you would like to preserve your child’s place on my tutoring schedule, a hold fee equal to 60% of your normal tutoring fees will be required. If you elect not to pay a hold fee while you are away, your space may be given to another family and I am unable to guarantee that your child will be able to resume tutoring with me once you return. During summer months, each family is allowed to miss two weeks of sessions without hold fees.
Termination of Services: It is requested that termination of services occur with 14-21 days advance notice or to allow 2-3 final sessions. This will enable smooth transition planning and a positive completion process to occur with your child.
Informed Consent: I understand that per Full Circle Math policy, all information, communications, observations, and opinions derived from this service will be kept confidential, but may be shared among professionals for the purpose of providing the highest quality service. I understand that educational services cannot be guaranteed to produce a specific outcome or result, including, but not limited to exam(s) scores of any kind or admission to a particular school or program.
