Sacred Heart Summer Activity Permission Slip
By completing this document, I give permission for my son/daughter to participate in Phase 1 of Sacred Heart Summer Activities/Athletics to the extent allowed by Sacred Heart Schools using Pettis County Health Department recommendations and NFHS Recommendations. This permission slip is required for participation for any student entering grades 7-12 and all graduating seniors wishing to participate.
Phase 1 Guidelines: Coaches will perform a temporal temperature scan at the beginning of each practice session. During Summer Sessions, athletes will be required to maintain the recommended social distancing guidelines of 6 ft. Balls and equipment will be sanitized at the beginning and end of each practice session. No scrimmages or contact drills will be allowed during phase 1 activities. All sessions and workouts are VOLUNTARY.
Any student with a temperature exceeding 100.0 will not be allowed to participate.
As a parent/guardian of a student-athlete, I agree to keep my student home if I can answer "yes" to any of the questions below:
1. Has anyone in your household, including your child, tested positive for Covid-19 in the past two weeks?
2. Has your student had a fever of 100.0 or higher within the last 24 hours?
3. Does your student have any of the following signs/symptoms of respiratory illness or Covid-19?
-A fever of 100.0 or greater**
-Shortness of Breath
-New Loss of Taste or Smell
**If your student has a fever, they will need to be fever-free for 72 hours without fever-reducing medication before returning to activities. If they don't have a fever but have any of the other symptoms, they should be symptom-free before returning to activities.
