Please Indicate your levels of participation then pay FDPA fee of $125 by Oct. 31, 2017, to secure your place in the event. Payment can be made on FDEO.org (http://www.fdeo.org/events/beyond) or by check payable to FDEO and mailed to Daniel Lewis, FDEO Treasurer at 8901 Southwest 79th Ct. Miami, FL 33156. All forms (FDPA registration, Class sign ups, Concert application/ video, and t-shirt orders) are due by Nov. 28, 2017. Any applications received late will go on a wait list. Payments made after the deadline must include a $25 late fee.
Beyond the Test: Mentoring for Artistic Engagement Refund Policy:FDEO will refund school FDPA registration and class fees minus a $25.00 processing fee for each event up until January 9, 2018. Request must be in writing. If the request is received in writing between January 9 and January 16, 2018, FDEO will retain the FDPA registration and will refund 50% of the class fees. No refunds after January 16, 2018. No refunds for event t-shirts unless order is cancelled in writing to firstname.lastname@example.org before January 9, 2018.
