Student Language Proficiency Exam Request Form (PSU Students)
This form is to be completed by students as the first step to organize testing to fulfill a language requirement. Once we receive this exam request form, we will email you approval to pay for and schedule the exam. Taking an exam without approval or payment may invalidate your exam.

TESTING DEADLINES:
  • Exam requests are accepted from the first day of classes through the Friday of Week 6. 
  • If you are submitting this form after Week 6, we will send you the link to pay for your exam at the beginning of the next academic term. 
  • Exams must be taken by the Friday of Week 8.

If you have any questions about how to complete this form, please contact your advisor.
  1. Please complete the form below
     •    You should have received an email with the name of the exam your advisor referred you to take when your advisor submitted the Advisor Language Proficiency Exam Referral Form. Please reference the information in that email to complete this form below.
      •  If your advisor has not yet submitted the Advisor Language Proficiency Exam Referral Form on your behalf, please email the following link to them: https://docs.google.com/forms/d/e/1FAIpQLSfC_GoSaH5tgin4R5gkLNgo9-dLY87pPt6NhF_aazXMpS7lQw/viewform. 
      •  Your assigned advisor's name and contact information is listed in your myPSU account on the "Academics" page.
  2. Once we receive this exam request form, we will email you approval to pay for and schedule the exam. Taking an exam without approval or payment may invalidate your exam.
  3. TESTING DEADLINES:
      •  Exam requests are accepted from the first day of classes through the Friday of Week 6. If you are submitting this form after Week 6, language proficiency exams will be offered during the next academic term.
      •  Exams must be taken by the Friday of Week 8.
  4. A copy of this completed form will be sent to you for your records.

  1. Please complete the form below and choose the "Non-Credit" test option.
  2. Once we receive this exam request form, we will email you approval to pay for and schedule the exam. Taking an exam without approval or payment may invalidate your exam.
  3. TESTING DEADLINES:
      •  Exam requests are accepted from the first day of classes through the Friday of Week 6. If you are submitting this form after Week 6, language proficiency exams will be offered during the next academic term.
      •  Exams must be taken by the Friday of Week 8.
  4. A copy of this completed form will be sent to you for your records.
Instructions for Fulbright Exams:
  1. Please complete the form below and choose the "Fulbright Language Proficiency Evaluation" test option in the language you speak.
  2. Once we receive this exam request form, we will email you approval to pay for and schedule the exam. Taking an exam without approval or payment may invalidate your exam.
  3. A copy of this completed form will be sent to you for your records.
Instructions for German Goethe Exams:
  1. Please complete the form below and choose the German "Goethe B1", "Goethe B2", or "Goethe C1" test option.
  2. Once we receive this exam request form, we will email you approval to pay for and schedule the exam. Taking an exam without approval or payment may invalidate your exam.
  3. A copy of this completed form will be sent to you for your records.
