Festival Center Questionnaire
The Festival Center is a long standing, faith-based non-profit located in the Adams Morgan neighborhood of Washington, DC.  It is rooted in a Christian tradition that values spirituality, hospitality, and social justice.

We will be hosting a Focus Group on Tuesday, May 23 at 6:30pm.  The Group will be held at the Festival Center which is located at 1640 Columbia Road NW  Washington, DC  20009.  

If you are unable to attend the Focus Group, please complete this brief questionnaire.  Please know that all of your information will be kept confidential and will not be shared with any outside party.  Also, the questions pertaining to gender, age, and race will be for internal use only.
